Beaver Ridge UMC

If you would like to register your child for one of our programs, forms for each program can be found below. Please, print the forms, fill them out, and deliver them to Beaver Ridge UMC Parent's Day Out and Preschool. Registration for each of the programs runs on a first, come first served basis, and the registration fee is due when you turn in your paperwork. Before printing the forms, please contact us to be sure we have an available spot in your child's class.
